If you need your Medical Record, signed by a UK Doctor apostilled, you can send it to us for apostille processing.
All Medical Records will be certified by our solicitor (where necessary) and have an official, FCDO-issued, apostille certificate attached within 3 working days (or sometimes just a few hours with e-Apostille).

Your document will be transported using Royal Mail's Guaranteed Special Delivery service to the FCDO Legalisation Office and back (where necessary).

What is a Medical Record?
A Medical Record could be your entire medical history, or a shortened, more digestible version of it.
Who can get a Medical Record?
A request for information from your Medical Records has to be made with the organisation that holds your records – the data controller. For example, your GP practice, optician or dentist.
For hospital records, contact the records manager or patient services manager at the relevant hospital trust.
